Documents for Tutors
Below you will find two important documents:
1. Tutor/Client Agreement---Once the Tutor/Client have established the subject matter to be taught, location or method of tutoring, and the meeting times, the Tutor will fill out their portion of the agreement and send it on to the Client. Once the agreement is completed a copy should be send to ParentalChoiceTutoring@gmail.com and the Tutor and Client should also keep a copy for their records.

2. Hour/Session Documentation---The Tutor will fill out this form for each client and submit it every two weeks based on the payroll schedule. The Tutor should get a written or verbal confirmation from the Client before submitting.
